Prefer to speak with a real person? No problem. Our friendly reception team is always happy to help! Book online Call our team 2 Complete New Patient Form Sent via SMS link! The day before your appointment, you’ll receive an SMS reminder with a secure link to complete our New Patient Form. The form collects some basic information, including your contact details, Medicare information, medical, lifestyle information and payment details. You won’t be charged when completing the form. Payment details are securely stored via Stripe and are only used to process your appointment fee after your consultation, for telehealth appointments, or in accordance with our cancellation and non-attendance policy. If the form isn’t completed, we’ll send a few reminders and contact you before your appointment. Unconfirmed appointments may be offered to another patient. If you have a valid Medicare card, Medicare rebates are available for most services. Patients with Overseas Student Health Cover (OSHC), travel insurance or other healthcare arrangements, may be able to claim through their provider. Our reception team is always happy to explain your options before you book. 5 Transferring Your Medical Records If you’d like your previous medical records transferred to Brunswick Doctors, we can help organise this for you. Simply complete a transfer form and we’ll contact your previous clinic on your behalf. For most routine appointments, we usually don’t need your previous medical records before your first visit. If your doctor requires additional information from a previous clinic, they will discuss this with you during your appointment. Please note that transfer times vary between clinics, and some practices may charge a fee to prepare and transfer your records.
