If you require a form filled, please inform our staff member or state that in your reason for visit when making an appointment.
Be sure to bring the correct form along with any supporting documentation with you! If you'd prefer, we can assist by printing the form for you. There's a nominal charge of $0.30 per page of the document.
Medical Certificate for work or school
If you are unable to work due to an illness or injury, you may require a medical certificate from a GP to confirm your condition. Although the certificate typically does not disclose the specific illness to protect your privacy, it informs your employer that you had a medical condition leading to your absence from work. This can help ensure that you receive the necessary benefits associated with sick leave and protect your job security.
It's important to note that medical certificates can only be issued on the day of your visit to our clinic and cannot be written retrospectively (can't be backdated).
So, if you need a medical certificate for work or school, you must attend the practice on the day that you're ill. The doctor will then evaluate your condition and decide whether to provide you with a certificate at their discretion.
Special consideration for School or University
If you're experiencing unexpected and unavoidable circumstances that are affecting your studies, such as physical or mental illness, injury, or bereavement, you can apply for academic support through the special consideration process. This might include requesting an extension on an assignment, submission deadline, rescheduling an exam, or adjusting an assessment task to better suit your needs.
Each university or school has its own process for applying for special consideration. Some may require specific forms to be filled out, such as the University of Melbourne and RMIT, while others simply require a letter from a doctor.
Regardless of the requirements, students are generally required to provide evidence of their circumstances, such as a medical certificate or other documentation from a GP doctor, and to submit their application within a specified timeframe.
To avoid delays, it's important to check with your school or university prior to your appointment and inform our staff members that you require special consideration. Be sure to bring along any supporting documentation!
Insurance Forms
Insurance providers such as Bupa, Allianz, TATA AIG, and NIB may require you to have a form filled out by a doctor before you can make a claim for coverage or benefits for various types of insurance, including health, travel, or life insurance.
It's important to familiarise yourself with the claiming process of your insurance provider before your appointment, as our doctors can only fill out the form during your visit.
Note that if you have an OSHC (Overseas Student Health Cover) policy, you generally do not need a form filled out by a doctor, but it's still a good idea to check with your insurance provider for more information.

Fit-to-Fly Certificates from a doctor

I need a medical certificate for a previous sick day, what should I do?
Medical certificates can only be issued on the day of your visit to our clinic and cannot be backdated. If you require a document to prove your inability to attend work due to illness or injury, you can complete a Statutory Declaration and submit it to your employer.
A Statutory Declaration is a legal document where a person declares that something is true to the best of their knowledge and belief. It is typically used when evidence or proof of a particular fact is required.
