We work alongside psychiatrists to ensure patients receive appropriate treatment and ongoing care. To get started, please book a long consultation with our doctor and state ' ADHD Referral' as your reason for the visit , or contact our team in advance so we can prepare everything for you. Below is a step-by-step guide to our ADHD referral and management process. Step 1: Initial ADHD Consultation What to expect: ADHD Questionnaire: You will be required to complete an ADHD questionnaire before your appointment. If you would like to fill it out before your appointment, contact our team and we can forward the questionnaire to you. Otherwise, you can complete it at the clinic. Medical & Mental Health History: Our doctor will conduct a thorough review of your medical history, mental health background, lifestyle factors and current medications. Pathology Request: If needed, our doctors may request additional testing to assist with the assessment.
